Si tiene un examen programado con uno de nuestros médicos, imprima y complete el siguiente paquete de información para pacientes nuevos. Lleve estos formularios completos a su cita junto con sus tarjetas de seguro y los anteojos o ayudas visuales que utilice actualmente. Si tiene alguna pregunta, comuníquese con la oficina donde está programada su cita.

Patient Portal

In order to securely communicate confidential Patient Health Information, a Patient Portal will be registered to you through our office. You will receive an automated email with your username and the following link to log in to the portal: click here

You will be asked to create a password and security question for future access. Please contact our staff if you forget your password or username, or if you need help logging into the portal. A summary of your low vision consultation will be uploaded to this Patient Portal, where you may view, download, and share the documents at your convenience.

Referencias de pacientes

Referring doctors: please fill out the Low Vision Evaluation Referral Form for patients who need to see our optometrists or the Occupational Therapy Referral Form for patients who need a home evaluation and/or training with low vision aids or assistive technology.
